The Northeast Health District’s Ryan White Part B program is seeking a qualified person to provide community HIV education, counseling, testing, and linkage to care. The selected candidate will be responsible for the implementation of the Anti-Retroviral Treatment and Access to Services (ARTAS) intervention. The Linkage Coordinator is responsible for marketing the intervention to community partners, performing data collection and entry activities, and uses the intervention to link HIV-positive individuals to care or to re-engage them back in care.
As a Minority Aids Initiative Linkage Coordinator within the Northeast Health District at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.
Job Responsibilities
You will support the Department by utilizing your expertise to fulfill the following responsibilities:
- Links HIV-positive individuals to care and re-engages HIV-positive individuals back into care.
- Collaborates with HIV Coordinators, surveillance, and quality management to identify patients who are lost-to-care and are unable to be brought back into care by the clinic staff.
- Develops a protocol for identifying patients who are lost-to-care and re-engaging them back into care.
- Investigate and locate patients who are lost-to-care, determine if and where they are engaged in care.
- Plans and coordinates delivery of intervention activities.
- Maintains records; develops and implements care plans.
- Maintains consistent communication and coordinates services and activities with HIV Coordinators.
- Prepares and submits monthly service reports and quarterly narratives.
- Meets with multi-disciplinary team on a scheduled basis.
- Participates in quality management meetings to collaborate, identify, and report on issues such as barriers to care.
- Other duties as assigned.
Minimum Qualifications
Master’s degree in public health education from an accredited college
OR
Bachelor’s degree in education, social, behavioral or biological sciences or a directly related field from an accredited college
Annual salary $42,000 plus generous benefits include 13 paid holidays, annual and sick leave, health insurance, and retirement plans.
