The Perinatal Health Partnership (PHP) Program provides home visiting services during pregnancy and up to the infant’s first birthday following delivery. Pregnant women with high-risk conditions or who have identified risk factors that put them at risk for poor pregnancy outcomes will be targeted for home visiting services and interventions, but enrollment can occur after delivery.

Currently, the Perinatal Health Partnership is available to residents of Clarke, Elbert, and Greene counties in the Northeast Health District.



The PHP program is recommended for:

People who are pregnant or who have recently given birth who have any of the following:

  • Hypertension or Gestational Hypertension (high blood pressure)
  • Preeclampsia
  • Diabetes
  • Multiple gestation (twins, triplets)
  • Prior preterm delivery (previous baby born premature)
  • Preterm labor
  • Chronic conditions or co-morbidities
  • Prior 2nd-trimester pregnancy loss (miscarriage)
  • Prior fetal/neonatal death
  • Pre-existing health conditions
  • Substance use disorder
  • Mental health condition
  • Lack of a strong support system
  • Barriers to follow-up care (for example, difficulty keeping appointments) or other difficulties with fully participating in a provider-recommended heathcare plan
  • Other medical conditions or concerns for poor outcomes

Infants with:

  • Recent NICU discharge
  • VLBW or LBW (low birth weight)
  • <36 weeks gestation at delivery (baby born prematurely)
  • Positive maternal screening for substances at delivery
  • Suspected or confirmed congenital syphilis or HIV infection
  • Poor familial support system or other environmental concerns

Perinatal Health Partnership services include:

  • Clinical assessment of pregnant or recently delivered individuals and infants in their first year of life
  • Care coordination
  • Case management
  • Education for high-risk pregnant individuals and their families
  • Linkages to needed resources
