We provide each of our patients with a copy of our Patient Rights and Responsibilities, as well as Grievance Policy.
As a part of our program guidelines, each of those documents is reviewed with you annually or bi-annually. This establishes a mutual understanding between our patients and providers.

Your Privacy is Important:

• We confirm your identity when you call.
• We will never divulge if you are our patient without your written consent – including to family and friends.
• All patient records and protected health information (PHI) are maintained under HIPAA compliant safety measures.
• Our clinic uses secure and discreet methods to send out messages to you by phone, text and mail.
